At Community Care Development Network (CCDN), we believe in walking alongside individuals and families as they build stronger, more stable lives. Our work is rooted in compassion, connection, and community. Through a holistic approach, we provide comprehensive services that address immediate needs while creating pathways for long-term success.

Human Support Services
Our Human Support Services Program is the cornerstone of CCDN’s outreach—meeting the most urgent needs of families across Jefferson County. Each month, we serve between 2,600 and 3,000 families by providing:

​

  • Access to fresh, nutritious food

  • Clothing for all ages and seasons

  • Emergency financial assistance

  • Shelter and housing support

  • Connection to critical resources and referrals

 

Whether it's a single mother needing diapers and formula or a senior adult requiring utility assistance, we’re here to respond with care and compassion.

 

Family Support Services
Strong families build strong communities. Our Family Support Services Program empowers both youth and parents through mentorship, education, and support. We offer:

​

  • Precious Pearls of Promise: A mentorship and life development program for young ladies ages 11–18

  • Diamonds in the Rough: A mentorship and life skills program for young men ages 11–18

  • Parents Helping Parents: A peer-led support group that strengthens parenting skills and builds community

  • Private Family Sessions: Tailored sessions for families navigating conflict, trauma, or transitional life challenges

These programs help build confidence, communication, and connection within families—laying the foundation for generational growth and healing.

Education Support Services
Through our Inspire Education Academy, we offer a second chance at success for youth and young adults ages 16 and up who have faced life barriers, including court involvement and incomplete schooling. The Academy provides:

​

  • High school diploma attainment

  • Career pathway development

  • Hands-on trade and workforce readiness programs

  • Life skills coaching and transition support

 

We believe every young person deserves the opportunity to succeed—Inspire helps them take the first step.

Community Outreach
Our outreach efforts extend beyond our walls and into the heart of the communities we serve. These initiatives include:

​

  • E.L.E.C.T. Teen Summit: An annual empowerment experience for youth, featuring workshops, speakers, and skill-building sessions

  • Back 2 School Supplies – Year-Round: Ensuring children have the tools they need to succeed every day, not just in August

  • Pop-Up Food Hubs: In partnership with local organizations, we bring food and resources directly into underserved neighborhoods

  • Collaborative Community Projects: We work with churches, schools, and local agencies to address shared goals and community needs
