Human Support Services 

CCDN’s largest program is its Human Support Services Program, which assists families with such as food, shelter, clothes, resource referrals, etc.).  The program serves approximately 2400-3000 families monthly.

At CCDN, we recognize that food is just one of many essential needs that families and individuals face daily. Our Human Support Services Program is an extension of our mission to ensure Prevention, Access, and Stability by addressing a broader spectrum of challenges that affect overall well-being.

 

Prevention: Responding to Urgent Needs Before Crisis Hits

  • When families lack basic essentials like hygiene products, diapers, clothing, or emergency assistance, the ripple effect can lead to long-term hardship. Our Human Support Services proactively address these needs to help individuals avoid deeper crises, providing a foundation for personal dignity and stability.


Access: Bridging the Gap to Essential Resources

  • We understand that navigating support systems can be overwhelming. Our program simplifies the process by offering one-stop access to critical supplies and services. Through CCDN’s Human Support Services, individuals receive personal care items, resource referrals, and access to wraparound resources—all designed to break barriers and foster independence. With the help of community partners, local agencies, and compassionate donors, CCDN distributes essential goods and services through coordinated events and ongoing outreach efforts. Whether through our Essential Needs Assessments or direct support services, we ensure that families receive the right help at the right time.

Community Food Hubs Program
